Why Upper Eyelids Sag
The role of muscle spasms in drooping upper eyelids
The overhang of excess skin on the upper eyelids is one of the most obvious signs of age-related changes. It is often believed that this excess skin forms due to atrophy of the circular muscle of the eye. However, in the vast majority of cases, the opposite occurs: almost along its entire perimeter, the circular muscle of the eye does not lose its tone but instead spasms, leading to the appearance of excess skin. Imagine an elastic band of underpants that, due to its smaller size, causes the fabric covering the band to sag.
Many people's eyes become smaller and seem to sink deep into their sockets. In cases of severe spasm of the circular muscle, the size of the eye decreases markedly with age. The deformation of the circular eye muscle can be likened to a narrowing of the camera diaphragm, whose blades curl along the upper eyelid toward the inner corner of the eye. Consequently, the skin of the upper eyelids becomes superfluous and folds, and the size of the eye itself decreases significantly.
The overhang of the upper eyelid is often provoked by complex deformations of not only the circular eye muscle but also the frontal and temporal muscles. The outer part of the circular eye muscle is woven into the frontal muscle at the top, so the position of the central part of the eyebrow directly depends on the condition of the forehead muscles. As it descends, the frontal muscle pulls the main part of the eyebrows, leading to the formation of excess skin. From the periphery, the outer part of the eyebrow is compressed by the temporal muscle—it causes the eyebrow to drop and provokes the appearance of excess skin hanging over the outer corner of the eye. The inner part of the eyebrow descends under the weight of a spasm of the muscle that wrinkles the eyebrow. There is nowhere left for the skin above the corner of the eye to go, so it begins to sag.
